How do I file a claim?
In order to receive benefits you must file an application (First Report of Injury
or FROI) with the Ohio Bureau of Workers' Compensation. A claim application may
be obtained from the employer or you can file a claim directly with BWC in person,
on-line or by mail. Upon filing the application, you will receive a claim number
from BWC. That number should be submitted to all medical providers so medical
bills can be sent directly to BWC for payment through your claim. After receiving
your application for benefits, BWC will then issue an order either granting or
denying the allowance of your claim. Either the injured worker or the employer
can file an appeal to the BWC order if they disagree with any portion of the order.
An appeal must be filed within 14 days or the decision of the BWC will become
final. What compensation can I expect to receive? You
may be eligible to apply for various types of compensation to make up for lost
wages while you are recovering from your injuries or for any permanent physical
or psychological impairments caused by the injury. If your injuries prevent you
from returning to work in any capacity, you may be eligible to file for a lifetime

Who is covered under the Workers' Compensation laws in Ohio? Generally,
any employee who sustains an injury in the course of his/her employment will be
eligible to file a claim for benefits with the state of Ohio or directly with
their employer. Is there a time limit within which to file a claim? Yes.
Generally, you have two years from the date of the injury within which to file
a claim. Failure to file a claim within two years of the date of injury may lead
to the claim being disallowed.
